The following forms are for continuing education students.

Certificate/Diploma Request Form

Please complete and submit the Continuing Education Certificate/Diploma Request Form via scan and email or by fax to Open Learning and Educational Support.

To drop a continuing education course, please complete and submit the Continuing Education Drop Form via scan and email or by fax to Open Learning and Educational Support.

Transcript Request Form

To request an official transcript, please complete the Transcript Request Form and fax or mail it to our main office:

Distance Education and Open Learning Program Students

Certificate Application Form

To request a certificate, complete the Certificate Application Form and fax or mail it to our main office.

Course Material Shipment Form

Additional materials (e.g., printed manuals, media, etc.) may accompany your DE course. These materials are included in your tuition fees and are available through OpenEd. If you live at a distance or can't make it to campus, fill in and submit the Course Material Shipment Form to request shipment at no extra charge.

Drop Forms

Open Learning program (OLp): OLp students can drop courses on or before the 40th class day each semester by filling in the Open Learning program Drop Form.

University of Guelph degree students: University of Guelph degree students can drop courses on or before the 40th class day each semester. This can be done online using WebAdvisor.

Final Examination Information Form

If you are studying at a distance, you can request to write your final exam at an alternate location. It is recommended that you make arrangements as early as possible in the semester since changes cannot be guaranteed after the deadline.

University of Guelph degree and associate degree students:

To make off-campus arrangements, please email the DE Exam Coordinator for late requests.

Open Learning program students:

Open Learning program students will request to write off campus through the OpenEd Student Portal by selecting "My Exams" in the student Home menu.

University of Guelph degree students: University of Guelph degree students can request transcripts from Enrolment Services. Please visit the Enrolment Services website for more details.
